All forms may be completed online or printed at home and completed. The registrar's office will provide printed forms upon request. When printed forms are complete you may choose from the following options:

• Scan and email the Application & Required Documents to the Registrar at: corina.nuby@mcdadeisd.net

• Hand Deliver the Application & Required Documents to Central Office: 156 Marlin Street, McDade, Texas

• Mail the Application & Required Documents to: Corina Nuby, Registrar P. O. Box 400 McDade, Texas 78650

Do not complete the New Student Registration forms if your child is a returning student or if you have completed a transfer or pre-kindergarten application that has not yet been approved by the district.

Registration Documents:

• Certified Birth Certificate of Student

• Copy of Student Social Security Card

• Current Immunization Record

• Copy of Parent/Guardian Current Driver’s License

• Proof of Residency (i.e., utility bill, lease agreement, or tax statement)

• Copy of Final Report Card (2023-2024 School Year) for Students in Grades 1st-12th
